Nursing care in patients with gastrostomy

Aim
Gastrostomy is used in individuals unable to receive oral nutrition due to neurological, genetic, or congenital pathologies, aiming to ensure adequate hydration, nutrition and administration of enteral medications. Nurses assume an integral role in educating patients with gastrostomies and their caregivers. Gastrostomy complications encompass tube dislocation, leakage, obstruction, and formation of granulation tissue. The purpose of our study is to evaluate the care processes of patients undergoing post-gastrostomy placement follow-up in the pediatric intensive care unit.
Methods
Between 2021 and 2023, data of patients who were followed in the Pediatric Intensive Care Unit of Trakya University Hospital after undergoing gastrostomy placement were retrospectively reviewed. Pre- and post-operative weight measurements, nutritional intake volumes, and gastrostomy-related complications of the patients were recorded.
Results
In the conducted assessment, 9 patients who were monitored in the pediatric intensive care unit after undergoing gastrostomy placement were identified. It was observed that the patients reached their targeted nutritional volumes and experienced weight gain following gastrostomy placement. The most frequently observed complications were tube blockage and its advancement into the stomach. Complications were resolved through the implementation of suspension dressings, the use of a gastrostomy tube fixable with clips, and the irrigation of the tube with warm saline before and after feeding. Prior to discharge, the family was provided with education regarding nutrition through gastrostomy and gastrostomy care.
Conclusions
Gastrostomy education requires a multidisciplinary study. Nursing care is an important part of this study. Thanks to good nursing care, the adaptation of the patient and the family to the gastrostomy tube increases, and gastrostomy-related complications decrease.
Gastrostomili hastalarda hemşirelik bakımı

Amaç
Nörolojik, genetik veya konjenital patolojiler nedeniyle oral yoldan beslenemeyen hastalarda yeterli hidrasyon, beslenme ve enteral ilaçların uygulanması için gastrostomi kullanılmaktadır. Gastrostomisi olan hastaların ve bakıcılarının eğitiminde hemşireler bütünleyici bir rol oynamaktadır. Gastrostomi komplikasyonları arasında tüpün yerinden çıkması, sızıntı, tıkanıklık ve granülasyon dokusu oluşumu sayılabilir. Çalışmamızın amacı gastrostomi açılması sonrası çocuk yoğun bakım ünitesinde takip edilen hastaların bakım süreçlerini değerlendirmektir.
Yöntem
2021 – 2023 tarihleri arasında gastrostomi açılması sonrasında Trakya Üniversitesi Hastanesi Çocuk Yoğun Bakım Ünitesi’nde takip edilen hastaların verileri retrospektif olarak tarandı. Hastaların operasyon öncesi ve sonrası tartı alımları, beslenme hacimleri, gastrostomi ilişkili komplikasyonları not edildi.
Bulgular
Yapılan değerlendirmede gastrostomi açılması sonrası çocuk yoğun bakım ünitesinde takip edilen 9 hasta saptandı. Gastrostomi açılması sonrası hastaların hedef beslenme hacimlerine ulaştıkları, tartı alımlarının olduğu görüldü. Gastrostomi tüpünün tıkanması ve mide içerisine ilerlemesi en sık görülen komplikasyonlardı. Askılı pansuman yapılması, klips ile sabitlenebilen gastrostomi tüpü kullanılması, beslenme öncesi ve sonrası sıcak serum fizyolojik ile tüpün irrige edilmesiyle komplikasyonlar giderildi. Taburculuk öncesi aileye gastrostomiden beslenme ve gastrostomi bakımı hakkında eğitim verildi.
Sonuç
Gastrostomi eğitimi, multidisipliner bir çalışma gerektirir. Hemşirelik bakımı, bu çalışmanın önemli bir parçasıdır. İyi bir hemşirelik bakımı sayesinde hastanın ve ailenin gastrostomi tüpüne adaptasyonu artmakla birlikte gastrostomi ilişkili komplikasyonlar da azalmaktadır.
