PERCUTANEOUS ENDOSCOPIC GASTROSTOMY: TECHNICAL CONSIDERATIONS

Aim: Percutaneous Endoscopic Gastrostomy (PEG) is a common minimal invasive alternative for the gastrostomy methods. In this video, we aimed to consider the technical details of the surgery.
Case Report: Four years old girl with spina bifida had feeding problems. Therefore she underwent PEG. Under general anesthesia, a flexible gastrostomy was performed. The guidewire of the PEG set was inserted under gastroscopic guideance into the stomach. With assistance of a flexible forceps put through the instrument channel of the gastroscope, teh guidewire was grasped and it was taken out of the mouth. The guidewire and the PEG catheter have been put together the guidewire has been taken out, and the catheter was inserted into the stomach. The control endoscopy verified the position of the PEG catheter. No complication has been encountered.
Conclusion: PEG is a relative simple and effective method for gastrostomy.

Amaç: Perkütan endoskopik gastrostomi (PEG), gastrostomi yöntemleri içinde sık kullanılan minimal invazif bir alternatiftir. Bu videoda PEG yapılan bir hastada operasyonun teknik detaylarının irdelenmesi amaçlandı.
Olgu sunumu: Dört yaşında kız hastaya spina bifidaya bağlı beslenme sorunları nedeniyle gastrostomi indikasyonu konuldu. Genel anestezi altında öncelikle bükülebilir gastroskopi yapıldı. PEG setinin kılavuz iğnesi ve bunun içinden de kılavuz tel karın dışından mideye gönderilerek, gastroskop enstruman kanalından geçirilen bir forseps yardımıyla tutuldu. Ağız dışına alınan kılavuz tel ile PEG kateteri birleştirilerek, karın dışındaki kılavuz telin ucu çekildi ve kateter mideye gönderildi. Kontrol endoskopi uygulanarak kateterin mideye yerleştiğinin sağlaması yapıldı. Komplikasyon olmadı.
Sonuç: PEG, oldukça kolay yapılan, komplikasyonu görece az ve etkin bir yöntemdir.
