Polydactyly of all four extremities

Polydactyly is the most prevalently encountered congenital anömaly of hands, and feet. It can be seen as an isolated anomaly or it can be associated with other congenital defects. Its most widely seen type is postaxial polydactyly. If it involves all of four extremities it is called tetrapolydactyly which is extremely rare.
A 22-year-old Syrian mother gave birth to her second children via spontaneous vaginal delivery at her 37. gestational week . The 28-day-old baby boy was referred to our polyclinic because of polydactyly of all of his four extremities On physical examination right hand ulnar postaxial polydactyly had only skin connection , while in her left hand, and both feet postaxial polydactyly was detected where fusion of the underlying bones is seen. Examination of other systems was unremarkable. Plain radiograms,, abdominal US, and echocardiograms were not pathologic. Laboratory test results were within normal limits As treatment rudimentary polydactyly of the right hand was ligated, supernumerary digits of the left hand, and both feet were excised, and the remaining soft tissues were reconstructed. Postoperative period was uneventful. Any congenital defect was not found in his first- degree relatives.
In this paper, an extremely rare sporadic case of postaxial tetrapolydactyly was reported. Even though our case was an isolated case with polydactyly, it should not be forgotten that this condition can be associated with a syndrome, and consequently in patients presenting with tetrapolydactyly, other systems should be also examined , and imaging modalities should be performed.

Polidaktili, el ve ayakların en yaygın rastlanan doğumsal anomalisidir. İzole olabileceği gibi diğer doğumsal defektlerle birliktelik gösterebilir. En yaygın görülen tipi postaksiyel olanıdır. Dört ekstremitede birden olduğunda tetrapolidaktili adını alan bu duruma son derece nadir rastlanılır.
Yirmiiki yaşındaki Suriyeli bir annenin 2. çocuğu olarak 37 haftalık spontan vajinal doğumla dünyaya gelen 28 günlük erkek bebek, dört ekstremitesinde birden polidaktili olması nedeni ile polikliniğimize başvurdu. Fizik muayenede, sağ el ulnar postaksiyel polidaktilide sadece cilt bağlantısı varken, sol el ve her iki ayakta kemik bağlantısı da olan postaksiyel polidaktili mevcuttu. Diğer sistem muayeneleri doğaldı. Direk grafi, abdomen USG ve EKO'da patolojiye rastlanmadı. Laboratuar testleri normal sınırlarda idi. Tedavide sağ eldeki rudimenter polidaktili ligate edilirken, sol el ve her iki ayaktaki fazla olan parmaklar eksize edilerek, geride kalan yumuşak dokular rekonstrükte edildi. Postoperatif dönem komplikasyonsuz idi. Birinci derece akrabalarında herhangi bir doğumsal defekt bulunmamakta idi.
Bu yazıda, son derece nadir bir durum olan postaksiyel tetrapolidaktilinin sporadik bir olgusu bildirilmiştir. Her ne kadar olgumuz, izole bir polidaktili vakası olsa da bu durumun bir sendroma eşlik edebileceği, bu yüzden tetrapolidaktili ile başvuran hastalarda diğer sistem muayeneleri ile görüntüleme yöntemlerinin mutlaka yapılması gerektiği unutulmamalıdır.
