Safe surgery saves lives

Aim: The World Health Organization Surgical Safety Checklist is designed to reduce the number of errors and complications resulting from surgical procedures by improving team communication. Using this checklist is becoming more common all of the World. Some countries and hospitals have their own local adaptations. The aim of the present study was to introduce our adaptation. Turkey's Health Ministry has added one more column to the list. This checklist has 4 columns: Before leaving the clinic, sign in, time out and sign out. In our hospital a schematic information and consent checklist is designed in additional to the WHO-Ministry of Health of Turkey checklist.
Methods: Front and back figures of the body are shown on the upper part of the form; the identity of the patient, patient's guardian and team have their own box to check and sign in the lower part of the form. During the admission, patient's guardian and surgeon, nurse in charge in the clinic, anesthesiologist in the operation room check and verify child's identity, site or/and side of the body, procedure and consent on the same schematic body diagram and sign their own box. The operation site and/or side are shown to family by drawing on the diagram during the verbal and written consent.
Results: From November 2009 to July 2015, the form was used by all surgeons who operated approximately 45000 patients (major-minor surgery) under general anesthesia. During this period of time, all operations were correctly verified by the family and team member on the form which is designed by the author.
Conclusions: Using this schematic body figures and consent boxes in addition to the safety checklist is safe, simple and quick. Especially it is useful for team communication. Easy to understand parent information and consent. Family and team have a high level of satisfaction.

Amaç: Dünya Sağlık Örgütü (DSÖ) Cerrahi Güvenlik Kontrol listesi, ekip iletişimini artırarak cerrahi işlemlerden kaynaklanan hata ve komplikasyon sayısını azaltmak için tasarlanmıştır. Bu listenin kullanımı tüm Dünya'da gittikçe yaygınlaşmaktadır. Bazı ülke ve hastanelerin kendi yerel uyarlamaları mevcuttur. Bu çalışmanın amacı bizim hastane uyarlamamızı sunmaktır. Türkiye Sağlık Bakanlığı(TSB), DSÖ' nün listesine 1 sütun daha eklemiştir. Bu kontrol listesi 4 sütundan oluşmaktadır: Klinikten ayrılmadan önce, anestezi verilmeden önce, ameliyat kesisinden önce ve ameliyattan çıkmadan önce. Hastanemizde ise DSÖ-TSB listesine ek olarak şematik bir bilgilendirme ve onay formu tasarlanmıştır.
Yöntem: Formun üst kısmında vücut ön ve arka şekilleri gösterilmektedir. Alt kısmında hastanın kimlik bilgileri, hasta sahibi ve ekibin isimleri ve onay kutucukları yer almaktadır. Yapılacak işlem ve ameliyat tarafı vücut şekilleri üzerinde aileye gösterilerek sözlü ve yazılı onam alınmaktadır. Başvuru anında hasta sahibi ve cerrah, klinikte sorumlu hemşiresi, ameliyathanede anestezi uzmanı; çocuğun kimliğini, aynı şekil üzerinde ameliyat bölge ve/veya tarafını, işlemi ve onamı kontrol ederek doğrular ve imzalar. Operasyon bölgesi ve/veya tarafı sözel ve yazılı onam esnasında aileye şekil üzerinde çizilerek gösterilir.
Sonuç: Bu form Kasım 2009 ve Temmuz 2015 tarihleri arasında tüm cerrahlar tarafından genel anestezi ile opere edilen yaklaşık 45000 hastanın (major-minor cerrahi) ameliyatlarında kullanılmıştır. Bu zaman aralığında tüm ameliyatlar yazar tarafından tasarlanan bu form üzerinde kontrol edilerek doğrulanmıştır.
Tartışma: Cerrahi güvenlik listesine ek olarak bu şematik figürler ve onay kutularının kullanımı emin, basit ve hızlıdır. Özellikle ekip iletişimi için yararlıdır. Aile bilgi ve onamı kolay anlaşılabilirdir. Aile ve takım memnuniyeti yüksek düzeyde olmuştur.
