A rare coexistence: Ureteral duplication and bladder diverticula

Introduction:
Coexistence of ureteral duplication and bladder diverticula is a rare condition. In this study we present a case of ureteral duplication anomaly opening into a bladder diverticula.
Case report:
In urinary ultrasound study of an 8 years old boy with hematuria, right sided grade III hydronephrosis and double collecting system were observed. Voiding cystourethrography revealed a right sided duplication and grade IV VUR in both systems. In DMSA scintigraphy, right differential function was 22%. During cystoscopy, right ureteral orifice was not observed, but it was found that there was a right sided bladder diverticula. When the scope was introduced into the diverticula, it was seen that the 2 orifices of complete duplicate systems had openings inside the diverticula. In retrograde contrast study, it was unexpectedly observed that the calibre of the ureter draining the non-functioning upper system was normal, but the ureter of lower system was extremely dilated. Laparoscopic heminephroureterectomy was performed to upper system. As a separate operation, bladder diverticula excision and Politano-Leadbetter ureteroneocystostomy were done. Postoperative follow up was uneventful in this patient.
Result:
In duplicate collecting systems, the ureter draining the non functioning moeity are generally dilated. The reported case is noteworthy because the above mentioned ureter was in normal calibre. Extreme care should be given for such variations during heminephroureterectomies. Cystoscopic ureteral catheterization prior to operation may enhance the safety of surgery.
Nadir görülen üreteral duplikasyon ve mesane divertikül birlikteliği: Olgu sunumu

Giriş
Üreteral duplikasyon ve mesane divertikülü birlikteliği az rastlanan bir durumdur. Bu çalışmada mesane divertikülü içine açılan çift üreter anomalisi sunuldu.
Olgu sunumu
Hematüri nedeniyle başvuran 8 yaşında erkek hastanın ultrasonografisinde sağ grade III hidronefroz ve çift sistem görüldü. İşeme sistoüretrografisinde sağda çift sistem ve her iki sisteme grade IV VUR saptandı. DMSA’ da sağ diferansiye fonksiyon %22 olarak ölçüldü. Sistoskopide sağda mesane divertikülü ve sağ çift sistem üreterlerinin divertikül içine açıldığı görüldü. Retrograd kontrastlı incelemede beklenenin aksine non-fonksiyone üst sistemi drene eden üreterin normal çapta, alt sistem üreterinin ise aşırı derecede dilate olduğu görüldü. Üst sisteme laparoskopik heminefroüreterektomi yapıldı. Ayrı bir seansta açık yöntemle mesane divertikül eksizyonu ve Politano-Leadbetter üreteroneosistostomi yapıldı. Hastanın ameliyat sonrası takibi sorunsuz seyretti.
Sonuç
Çift sistem varlığında non-fonksiyone sistemi drene eden eden üreterin genelde dilate olmasına karşın bu vakada normal çapta olması fonksiyone sistemi drene eden üreterin dilate olması orijinal bulunmuştur. Heminefroüreterektomide bu tür varyasyonlara dikkat edilmelidir. Ameliyat öncesi sistoskopik olarak kateterize edilen üreter güvenli cerrahiyi kolaylaştırır.
