Bleomycin treatment of postoperative and cosmetic concerns in the case of giant cystic lymphangioma in the neck
Aim: Cervical cystic lymphangioma is a rare disease usually be large and tend to cause respiratory distress. Cystic neck lenfanjiom a baby is born respiratory distress to attract underwent emergency surgery.Cosmetic concerns during the operative excision and postoperative seroma in developing treatment for patients with Bleomycin was presented.
Case: Prenatal diagnosed as having cystic mass in the neck term baby caesarean sectionborn full-with male patients,at birth due to respiratory distress, he was taken to intensive care.USG also approximately 11x5 cm, showing true extent of the area left cervical neck properly due to limited content is intense anechoic cystic lesion with cause respiratory distress so that underwent emergency surgery.Transverse incision made in the neck was excised total cyst. Pathology results came cystic lymphangioma. Postoperative follow-up no problem in the patient was discharged after the use of antibiotics during one week.When the patient came to control seroma is accumulated under the skin was drained from the edge of the incision also it was observed in skin appearance due to the wrinkled skin excision made in the operation. Three day intervals due to continuous seroma buildup despite the draining three weeks underwent a course of bleomycin treatment after four week. Seroma buildup after treatment was discontinued abruptly.The patien was not any problems and recurrence 6month follow-up. Cosmetic appearance, the skin improved with gain weight as the child grows markedly .
Conclusion: Cystic lymphangioma sometimes due to their large; After complete excision lived postoperative cosmetic results and seroma creates anxiety. Bleomycin treatment in spite of exceeding seroma accumulation can be applied to the drain .The skin excision is not necessary to conduct the operation. Cosmetics showed that receiving a bad image of the child's weight and grows markedly improved .

Boyunda dev kistik lenfanjiom olgularında postop Bleomisin tedavisi ve kozmetik kaygılar
Amaç: Boyunda kistik lenfanjiom nadir görülen bir hastalık olup genelde büyük boyutlu olmakta ve solunum sıkıntısı yaratmaktadırlar. Burada boyunda kistik lenfanjiomlu doğan bir bebeğin doğar doğmaz solunum sıkıntısı çekmesi nedeniyle acil operasyona alındı.Total eksizyon sonrası operasyon esnasında ki kozmetik kaygılar ve postop gelişen seroma tedavisinde Bleomisin kullanımı olgusu sunulmuştur.
Olgu: Antenatal olarak boyunda kistik kitle tanısı olan ve sezeryan ile miad erkek bebek olarak doğan hasta, doğar doğmaz solunum sıkıntısı olması nedeniyle yoğun bakıma alındı. Yapılan USG de boyunda sol servikal bölgede sol mandibuler ve infraauriküler bölgeye doğru uzanım gösteren yaklaşık 11x5 cm boyutlarında düzgün sınırlı içeriği yoğun anekoik kistik lezyon olması ve solunum sıkıntısı yaratması nedeniyle acil operasyona alındı. Boyundan yapılan transvers kesi ile kist total olarak eksize edildi.Patoloji sonucu kistik lenfanjiom ile uyumlu geldi.Postop takiplerinde sorunu olmayan hasta bir haftalık antibiyotik kullanımı sonrasında şifa ile taburcu edildi. Hasta kontrole geldiğinde cilt altında seroma birikmiş olup insizyon kenarından drene edildi ve operasyonda cilt eksizyonu yapılmaması nedeniyle de cilt buruşuk görünümde izlendi. 3 gün aralıklarla 3 hafta boyunca drenaja rağmen sürekli seroma birikmesi nedeniyle 4. haftada bir kür Bleomisin tedavisi uygulandı. Tedavi sonrası seroma birikmesi aniden kesildi. Hastanın 6 aylık takibindede herhangi bir sorun ve rekürrens olmadı. Cilltteki kozmetik görüntü de kilo aldıkça ve çocuk büyüdükçe belirgin düzeldi.
Sonuç: Kistik Lenfanjiomlar boyut olarak bazen çok büyük olmaları nedeniyle total eksizyon sonrası postop Seroma ve kozmetik sonuç kaygısı yaratmaktadır. Drenaja rağmen geçmeyen seroma birikimlerinde Bleomisin tedavisi uygulanabilinir. Operasyonda cilt eksizyonu yapılmasının gerekli olmadığı ve kozmetik kötü görüntünün çocuk kilo aldıkça ve büyüdükçe belirgin düzeldiği görülmüştür.
