A Serious Circumcision Complication: İatrogenic Proximal Hypospadias

Introduction:
Circumcision is the most frequent surgical operation of childhood. Post-circumcision iatrogenic proximal hypospadias is a very rare complication. In this paper, we present a two-year-old male patient with severe penile and urethral trauma after circumcision and with iatrogenic proximal hypospadias.
Case:
Two-year-old male patient presented to the emergency service of our hospital 5 days after his circumcision in Syria, with complaints of wound and discharge in his penis and urinating downwards. Physical examination revealed that the patient had severe tissue defect starting from the glans penis inferior and reaching up to proximal penile level and including the ventral lateral sides of the urethra, and iatrogenic proximal penile hypospadias. Patient was prescribed with antibiotic (ampicillin sulbactam) and nonsteroidal anti-inflammatory drug for one week. Definitive operation was scheduled for the sixth month.
Conclusion:
In inexperienced hands and under circumstances where the hygiene conditions are not sufficient, severe complications such as urethral and penile trauma can be seen, although rarely. Management of iatrogenic proximal hypospadias is hard and recurrence rate is high. Therefore, we think that preventing the development of such complications after circumcision is as important as treating them.
Ciddi Bir Sünnet Komplikasyonu: İatrojenik Proksimal Hipospadias

Giriş:
Sünnet çocukluk yaş grubunda en sık ve yaygın yapılan cerrahi işlemdir. Sünnet sonrası iatrojenik proksimal hipospadias çok nadir bir komplikasyondur. Bu makalede sünnet sonrası ciddi penil ve üretral yaralanması olan ve iatrojenik proksimal hipospadiası gelişen iki yaşında bir erkek hastanın sunulması amaçlanmıştır.
Olgu:
iki yaşında bir erkek hasta Suriye' de yapılan sünnet sonrası postoperatif 5. günde peniste yara, akıntı ve alttan işeme yakınması ile hastanemiz acil servisine başvurdu. Hastanın fizik muayenesinde glans penis inferiorundan başlayıp proksimal penil seviyeye kadar uzanan, üretranın ventral lateral yüzlerini de içine alan ciddi doku defekti, iatrojenik proksimal penil hipospadiası mevcuttu. Hastaya bir hafta antibyotik (ampisilin sulbaktam) ve nonsteroid antiinflamatuar ilaç tedavisi verildi. Altıncı ayda definitif operasyonu planlandı.
Sonuç:
Tecrübesiz ellerde ve hijyenin yeterli olmadığı koşullarda nadir de olsa sünnet sonrası üretral ve penil yaralanma gibi ciddi komplikasyonlar görülebilmektedir. İatrojenik proksimal hipospadiasın yönetimi zor ve nüks oranı yüksektir. Dolayısıyla sünnet sonrası bu tür komplikasyonların gelişmesinin önlenmesinin en az tedavisi kadar önemli olduğunu düşünüyoruz.
