Central Venous Catheterization With Wireless Ultrasonography Probe And Smart Glasses: A Pilot Application

Aim: Central venous catheter insertion is a surgical procedure that is frequently required, especially in neonatal and pediatric intensive care units and oncology clinics. The gold standard of the procedure is ultrasound guided percutaneous catheterization. One of the most challenging situation is to see the sonographic vision of juguler vein and insert the catheter needle at the same time. Since the plane of ultrasound monitor and juguler vein is not straight, the insertion may be quite difficult especially in newborns and small babies. In this report it was aimed to present the feasibility of smart glasses with the wireless ultrasonography device which brings the juguler vein and sonographic view in the same plane with bringing the sonographic view into the screen of the smart glass.
Method: Central venous catheter insertion was planned for a 21 days old 1880 gram baby since he had to receive medication for pulmonary hypertension. A 3 French central venous catheter was chosen regarding the weight of the baby.
Result: This device was used in the operating room by the surgeon, who had experience in applying percutaneous central venous catheters with the help of ultrasonography. The catheterization was performed in 17 seconds on first attempt and total procedure was performed in less than five minutes. There was not any complications.
Conclusions: With new technological developments, devices developed in a shorter time and safely in small babies can be used and facilitate the interventions to be applied.
Kablosuz Ultrasonografi Probu Ve Akıllı Gözlükle Santral Venöz Kateterizasyon: Pilot Çalışma

Amaç: Santral venöz kateter uygulamaları özellikle yenidoğan, çocuk yoğun bakım ve onkoloji hastaları için sıklıkla ihtiyaç duyulan cerrahi bir işlemdir. İşlemin altın standardı ultrason eşliğinde perkütan kateterizasyondur. En zorlayıcı durumlardan biri aynı anda hem juguler venin sonografik görüntüsünü görmek, hem kateter iğnesinin ponksiyonunu eş zamanlı uygulamaktır. Ultrason monitörünün ve juguler venin düzlemi düz olmadığı için özellikle yenidoğan ve küçük bebeklerde kateter yerleştirme işlemi oldukça zor olabilir. Bu çalışmada, ven ve sonografik görüntüyü aynı düzlemde buluşturan kablosuz ultrasonografi cihazı ile akıllı gözlüğün uygulanabilirliğinin ortaya konulması amaçlandı.
Yöntem: 21 günlük 1880 gram ağırlığındaki bebeğe pulmoner hipertansiyon nedeniyle ilaç tedavisi alması nedeniyle santral venöz kateter takılması planlandı. Bebeğin ağırlığına göre 3 French santral venöz kateter seçildi.
Bulgular: Daha önce ultrasonografi yardımı ile perkutan santral venöz kateter uygulama konusunda deneyimi olan cerrah tarafından ameliyathanede bu cihaz kullanıldı. Kateterizasyon 17 saniyede ve ilk denemede gerçekleştirildi ve toplam işlem beş dakikadan daha kısa sürede gerçekleştirildi. Herhangi bir komplikasyon olmadı.
Sonuçlar: Yeni teknolojik gelişmeler ile birlikte daha küçük bebeklerde daha kısa sürede ve güvenli olarak geliştirilen cihazlar kullanılabilir ve uygulanacak girişimleri kolaylaştırabilir.
