APPLICATION OF TESTIS FIXATION IN UNDESCENDED TESTICULAR SURGERY

Undescended testis is a common congenital anomaly in childhood, which is expressed as the failure of one or both testicles to descend into the scrotum. Orchiopexy is recommended and applied in order to provide a normal genital appearance, to prevent testicular torsion or incarcerated inguinal hernia, to increase fertility and to reduce the risk of malignancy. Although experimental studies have shown that adequate fixation can be achieved with a dartos pouch alone, many surgeons prefer to use one or more absorbable or non-absorbable sutures through the tunica albuginea to achieve secure fixation. Although different techniques have been proposed over the years for the most appropriate fixation, it is still controversial whether the fixation method is necessary. With this video presentation, we aimed to discuss testicular fixation applications with the current literature.

İnmemiş testis, testislerden birinin veya her ikisinin skrotuma inmemesi olarak ifade edilen, çocukluk çağında sık görülen bir doğumsal anomalidir. Normal genital görünümün sağlanması, testis torsiyonu veya inkarsere kasık fıtığının önlenmesi, doğurganlığın arttırılması ve malignite riskinin azaltılması amacıyla orşiopeksi önerilmekte ve uygulanmaktadır. Deneysel çalışmalar yeterli fiksasyonun tek başına dartos poş ile sağlanabileceğini gösterse de, birçok cerrah güvenli bir fiksasyon elde etmek için tunica albuginea'nın içinden bir veya daha fazla emilebilir veya emilemeyen sütür kullanmayı tercih eder. En uygun tespit için yıllar boyunca farklı teknikler önerilmiş olsa da tespit yönteminin gerekli olup olmadığı halen tartışmalıdır. Bizde bu video sunumu ile testiküler fiksasyon uygulamalarının güncel literatür ile tartışılmasını amaçladık.
