A CAUSTIC GASTRO DUODENAL STENOSIS . A CASE REPORT

Ingestion of caustic substances is very common in the pediatric population.
It is most often accidental; however, in adolescents, it is more frequently intentional.
It can cause severe injuries to the upper gastrointestinal tract.

We report the case of a 14-year-old girl who voluntarily ingested an acidic substance.
After three months of evolution, the patient presented with vomiting, dehydration, and severe malnutrition.
A significant decline in weight curve was noted, reaching -3 standard deviations.

Biological workup revealed mild hypoproteinemia and hypoalbuminemia, with no electrolyte imbalances.
Endoscopy showed an impassable antropyloric stricture.
A barium swallow (TOGD) revealed a reduced gastric capacity with a long antropyloroduodenal stricture at D1, followed by a shorter stricture at the D2-D3 junction.

Surgical intervention consisted of a stricturoplasty for the short duodenal stricture and a retrocolic gastrojejunostomy bypassing the long gastroduodenal stricture.

After four years of follow-up, clinical, biological, and radiological evaluations were satisfactory.

Caustic ingestion remains a significant public health issue in developing countries, posing considerable challenges regarding both therapeutic management and the financial burden associated with treatment.
The choice of surgical technique depends on the type and extent of the lesion.
Prognosis is determined by the severity of the injuries and their local and systemic repercussions.
Prevention through widespread awareness campaigns remains the only effective approach to addressing this issue.
