THE EFFECT OF AGE AND VUR ON THE REDUCTION OF RENAL PELVIC ANTEROPOSTERIOR DIAMETER IN THE FOLLOW-UP AFTER PYELOPLASTY

Purpose: In this study, the effects of age and vesicoureteric reflux (VUR) on the reduction of renal pelvic anteroposterior (AP) diameter following pyelopasty for ureteropelvic junction (UPJ) obstruction were evaluated.
Materials and Methods: Hospital records of 108 patients with UPJ obstruction who had undergone successful pyeloplasty between 2005-2014 were reviewed retrospectively. Two cases, operated twice for failed pyeloplasty were excluded. Two groups were created as; 75 patients < 2 years old and 33 patients >2 years old, and their at least three repeat ultrasound examinations (latest preoperative, one month after stent removal, and latest postoperative) were compared. The same analyses were performed separately for 10 patients with ipsilateral VUR.
Results: The mean age of 108 cases was 29,5 (±40,8) months. There were 22 girls and 86 boys. The mean age for the < 2 years Group was 6,8 (±6,1) months and for the >2 years Group 81 (±39,1) months.
The mean values for the studied parameters are depicted in the tables below.

Conclusion: After successful pyeloplasty, the AP diameter decreases approximately 50% at second postoperative month independently from ipsilateral VUR or the age of patient. The absence of expected reduction in pelvic size cannot be explained by age or coexistent VUR, and indicates questioning surgical success.
PYELOPLASTİ SONRASI İZLEMDE YAŞ VE VUR’UN RENAL PELVİS ANTEROPOSTERİOR ÇAPININ AZALMASINA ETKİSİ

Amaç: Pyeloplasti sonrası başarı renal pelvis anteroposterior (AP) çapının gerilemesi ile kantitatif olarak ölçülebilmektedir. Bu çalışmada üreteropelvik bileşke (ÜPB) obstrüksiyonu nedeniyle pyeloplasti uygulanan olgularda yaş ve vezikoüreteral reflünün (VUR) ameliyat sonrası AP çap gerileme sürecine olan etkisi değerlendirilmiştir.
Gereç ve yöntem: 2005-2014 yılları arasında başarılı cerrahi tedavi uygulanan 108 ÜPB obstrüksiyonu tanılı olgu geriye dönük değerlendirildi. Başarısız pyeloplasti sonrası tekrar opere edilen 2 olgu dahil edilmedi. Olgular 75’i <2 yaş, 35’i >2 yaş şeklinde iki gruba ayrılarak tekrarlayan en az üç ultrasonografileri (preoperatif en son, stent çıkarılması sonrası 1. ay, ve postoperatif en son) karşılaştırıldı. Aynı değerlendirme ipsilateral VUR’u olan 10 hasta için ayrı olarak yapıldı.
Bulgular: Yirmi ikisi kız, 86’sı erkek 108 hastanın ortalama yaşı 29,5 (±40,8) ay idi. <2 yaş grubunda ortalama yaş 6,8 (±6,1) ay, >2 yaş grubunda ise 81 (±39,1) ay bulundu.

Sonuç: Başarılı pyeloplasti sonrası ikinci aydaki ultrasonda AP çap hastanın yaşından ya da ipsilateral VUR varlığından bağımsız olarak, yaklaşık %50 azalmaktadır. AP çapta beklenen azalmanın görülmemesi ameliyat yaşı veya eşlik eden VUR ile açıklanamaz, bu durumlarda operasyon başarısının sorgulanması gerekir.
